Lila,

 

Please find attached the questionnaire you requested for the Xmas meeting
for December 13th.  I have formatted all the questions to a single page.  I
think I have made it quite obviously what answers we are looking for.
Please review and provide feedback if necessary.

 

I have CC'd executive with questionnaire for their feedback.

 

If everyone is happy with this form, I think it would be good to use when
calling those that do not provide answers during Xmas meeting.
